Abstract
Background: Failure to recognize clinical deterioration remains a global systemic challenge leading to preventable hospital mortality.

Aims: This study evaluated the multifactorial determinants predicting the accuracy of Early Warning System (EWS) implementation among clinicians in a regional hospital setting.

Methods: An analytical observational study with a cross-sectional design was conducted in 2025 at RSUD Perdagangan, Indonesia, involving 203 health professionals (specialists, general practitioners, midwives, and nurses) using total sampling. Data were collected via validated questionnaires and analyzed using Chi-square tests.

Results: Statistical analysis revealed that knowledge (p=0.001), training (p=0.002), attitude (p=0.005), and organizational support (p=0.001) significantly correlate with EWS implementation accuracy. Notably, 77.1% of clinicians failed to implement EWS effectively in environments lacking managerial support.

Discussion: These findings highlight that EWS precision is a manifestation of cognitive competence integrated with systemic organizational reinforcement.

Conclusion: Bridging the "accuracy gap" requires a paradigm shift from administrative compliance to a robust patient safety ecosystem that integrates structured training and active managerial oversight.</text>

<text>Hypertension is an increase in systolic blood pressure of more than 140 mmHg and
diastolic blood pressure of more than 90 mmHg in two measurements with an interval of
five minutes in a resting/calm state. This research is a quasi-experiment research with a
two group pretest and posttest design. The population in this study was 50 elderly people
in Bangun Rejo Village in 2024. The sampling technique was carried out using a
purposive sampling technique so that the total sample was 50 people. Data analysis used
the Wilcoxon test. The results of this study show that from the results of the excellent
innovation research, the acceleration of blood pressure reduction in hypertensive elderly
with deep breathing and a combination of natural ingredients (apium graveolens and
black garlic) means that the average blood pressure in the control group at the first
examination was 146.12 mmHg and at the second examination The average blood
pressure is 149.20 mmHg with a p value (mean) of 0.238 > 0.05, meaning it is not
significant. It was concluded that research on Excellent Innovation Accelerated Blood
Pressure Reduction in Hypertensive Elderly People Using Slow Deep Breathing and a
Combination of Natural Ingredients (Apium Graveolens and Black Garlic) in Bangun
Rejo Village in 2024 on 50 elderly people, with 25 elderly people as controls and 25
elderly people as cases, obtained results in the elderly as controls, the average blood
pressure at the first examination was 146.12 mmHg, while the average blood pressure at
the second examination was 147.28 mmHg. It can be seen that the difference in the mean
value between the first and second measurements is -1.26. The statistical test results
showed a p value = 0.238 > 0.05, which means there was no significant difference
between blood pressure in the first and second measurements in elderly people as
controls. Meanwhile, in elderly cases, the average blood pressure result of the first
examination was 165.72 mmHg, while the average blood pressure at the second
examination was 136.92 mmHg.</text>

<text>Introduction Surgery is a treatment action that involves making an incision or making a wound in body tissue, with the intention of removing a certain part of the body as a treatment. Operation. Laparotomy is a surgical procedure that involves opening the abdominal wall and chest wall to reach the contents of the abdominal cavity. The digestive surgical procedures that are often performed using a laparotomy incision technique are: herniotomy, gastrectomy, cholecystotomy, hepaterectomy, appendectomy, colostomy, hermorodectomy, cholecystomy, fistulectomy, or fistulectomy . To determine the relationship between early mobilization and the length of wound healing after laparotomy surgery at the Melati Perbaungan General Hospital. 2020. Design Correlative descriptive type research with cross sectional research design cross sectional where data between the dependent variable (early mobilization) and the dependent variable (length of wound healing) are measured at the same time, but not all research subjects are observed on the same day or time . Research methods The data collection method uses primary data, namely the type of questionnaire on the sample. Secondary data is data that supports this research such as health center profiles and annual reports. Results From the statistical test results , the value of p=0.000 was obtained , which means there is a significant relationship between early mobilization and wound healing in patients after laparotomy surgery. Meanwhile, the OR value = 0.067, meaning that respondents who do not carry out
early mobilization are likely to have a 0.067 times greater risk of slow healing compared to
those who carry out early mobilization. Conclusion Of the 30 respondents, the majority carriedout early mobilization, namely 18 people (60%), who experienced rapid healing of wounds after laparotomy surgery, namely 16 people (53.3%) Suggestions Researchers are expected to seek more knowledge about early mobilization and laparotomy .</text>
