                <text> Pasien post operasi bedah mayor beresiko tinggi mengalami komplikasi, terutama gangguan fungsi paru dan memperpanjang lama rawat inap. Pencegahan komplikasi ini dapat dilakukan dengan intervensi sederhana namun efektif berupa mobilisasi dini. Tujuan Penelitian: Penelitian ini dirancang untuk mengetahui efektivitas mobililisi dini dalam mengurangi lama perawatan di rumah sakit dan meningkatkan fungsi paru-paru pada pasien yang telah menjalani operasi besar di RSUP H. Adam Malik. Metode Penelitian: Pendekatan kelompok kontrol pretest-posttest diterapkan dalam studi ini melalui penggunaan desain kuasi-eksperimental. Prosedur bedah pascaoperasi besar dilakukan pada 60 pasien. Ada dua kelompok pasien: kelompok kontrol (30 pasien) dan kelompok intervensi (30 pasien). Setelah operasi, kelompok intervensi dimobilisasi lebih awal daripada kelompok kontrol, yang dimobilisasi sesuai dengan protokol standar rumah sakit dalam 24 jam pertama. Rekam medis digunakan untuk mendokumentasikan durasi rawat inap, dan fungsi paru-paru dievaluasi menggunakan spirometri (nilai FEV, dan FVC) Hasil Penelitian: Kelompok mobilisasi dini menunjukkal perbaikan fungsi paru yang signifikan secara statistik (FEV, dan FVC) dibandingkan dengan kelompok kontrol (p&lt;0,05). Selain itu pasien dalam kelompok intervensi mengalami masa rawat inap yang secara signifikan lebih pendek dibandingkan dengan kelompok kontrol (p&lt;0,01). Kesimpulan: Peningkatan fungsi paru-paru dan pengurangan durasi rawat inap keduanya merupakan keuntungan dari mobilisasi dini setelah prosedur bedah yang signifikan. Jenis intervensi ini harus diintegrasikan ke dalam protokol perawatan pascaoperasi statlar, perawatan pascaoperasi standar.</text>

Self-management support (SMS) is a patient empowerment strategy aimed at enhancing independence in nursing care. This study aimed to analyze the effectiveness of SMS on the quality of life of breast cancer patients at Haji General Hospital Medan. Methods: This study employed a quasi-experimental design with a pre–post test control group approach. The sample consisted of 60 respondents (30 in the intervention group and 30 in the control group), selected using purposive sampling. The intervention group received structured education and self-monitoring guidance for 8 weeks. The research instrument used was the EORTC QLQ-C30 questionnaire. Data were analyzed using Paired T-test and Independent T-test. Results: There was a significant increase in the mean quality of life score in the intervention group, from 52.3 to 75.6 (p &lt; 0.001), while the control group showed no significant change (p = 0.125). Intergroup comparison after the intervention revealed a statistically significant difference (p = 0.000). The emotional functioning and pain management domains showed the most substantial improvements. Conclusion: Self-management support is effective in improving the quality of life of breast cancer patients. This intervention is recommended to be integrated into standard oncology nursing care to promote patient independence in managing treatment-related side effects.</text>
