Robekan ini dapat terjadi selama atau setelah melahirkan, dan robekan ini bisa begitu menyakitkan. Menurut data World Health Organization (WHO) pada tahun 2019 jumlah Angka Kematian Ibu (AKD) sebesar 228/100.000 Kclahiran Hidup (KH) dan mengalami peningkatan sebesar 359/100.000 KH. Angka ini mash tinggi bila dibandingkan dengan target Sustainable Development Goals (SDG'S) tahun 2019 AKI sebesar 306/100.000 KH (WHO, 2019). Pada tahun 2020 di ketahui di Indonesia angka kejadian ruptur perineum pada ibu bersalin di alami oleh 83% ibu melahirkan pervaginam, ditemukan dari total 3.791 ibu yang melahirkan spontan pervaginam, 63% ibu mendapatkan jahitan perineum yaitu 42% karena episiotomi dan 38% karena robekan spontan (Kemenkes RI, 2021). Menurut Dinas Keschatan Provinsi Sumatera Utara tepatnya di kota Medan (2020), melaporkan 202 kematian ibu, yang terbagi menjadi 53 kematian ibu, 62 kcmatian ibu, dan 62 kematian ibu. Kelompok umur yang bertanggung jawab atas banyaknya kematian ibu adalah kelompok umur 20-34 tahun. 75% wanita yang melahirkan pervaginam mengalami ruptur perineum. dari total 1951 persalinan pervaginam alami, 57% ibu mengalami penutupan perineum (28% karena episiotomi dan 29% karena robekan alamil. 19% mengalami ruptur perineum, partus lama 11%, perdarahan masing-masing 10% dan eklampsia (Kemenkes, 2020). Dari hasil penelitian yang dilakukan oleh penulis dengan judul Asuhan Keperawatan Maternitas Post Partum Spontan pada Ny.N Dengan Nyeri Akut Ruptur Perineum Derajat II di Klinik Pratama Rawat Inap Santi Meliala Medan Tahun 2023 di dapatkan data bahwa 27 dari 35 ibu melahirkan mengalami ruptur perineum. Maka dengan itu penulis tertarik untuk mengangkat kasus ruptur perineum sebagai study kasus untuk menyusun dan memberikan Asuhan Keperawatan Maternitas pada ibu yang mengalami ruptur perineum. Metode penelitian yang di gunakan adalah jenis studi kasus yang bertujuan untuk mengeksplorasi langsung masalah yang di alami oleh klien dengan Diagnosa Nyeri Akut Ruptur Perineum di Klinik Pratama Rawat Inap Santi Meliala Medan di dapatkan kesimpulan yaitu : Setelah melakukan Asuhan Keperawatan Maternitas Post Partum Spontan pada Ny.N Dengan Nycri Akut Ruptur Perineum Derajat II di Klinik Rawat Inap Santi Meliala Tahun 2023 selama empat hari menggunakan 5 cara melakukan kebersihan pada area vulva, Ny. N mampu mengatasi nyer yang muncul pada saat melakukan mobilitas. Program pengabdian masyarakat ini bertujuan untuk meningkatkan kesiapan ibu hamil trimester ketiga menghadapi persalinan melalui pembelajaran mandiri berbasis e-book edukasi. Kegiatan pengabdian masyarakat ini berlangsung di PMB Shinta, Kecamatan Medan Johor, Kota Medan, melibatkan tiga puluh dua calon ibu. Kegiatan meliputi diskusi kelompok kecil, konseling, pembagian e-book, serta pembagian e-book, serta pemeriksaan prenatal dan postnatal untuk evaluasi. Hasil dari pengabdian masyarakat ini menunjukkan bahwa para peserta kini lebih memahami dan siap menghadapi proses persalinan. Ibu hamil dapat memperoleh pengetahuan dasar yang dibutuhkan sebelum melahirkan melalui publikasi elektronik ini. Tujuan dari inisiatif pengabdian masyarakat ini adalah untuk meningkatkan kesiapan ibu hamil trimester ketiga menghadapi persalinan melalui pembelajaran mandiri berbasis e-book edukasi.
