Pregnancy, especially in the third trimester, is a critical phase marked by significant physiological and psychological changes. Anxiety before childbirth is common among pregnant women and, if not properly managed, may lead to complications such as prolonged labor, uterine dysfunction, and increased obstetric interventions. This community service project aimed to reduce prenatal anxiety through the implementation of a diaphragm breathing exercise program for third trimester pregnant women in Bangun Rejo Village, Deli Serdang Regency. The program adopted a participatory educational approach involving 15 pregnant women who met the inclusion criteria. The activities included health education, demonstration, guided practice, monitored independent training, and evaluation. Knowledge improvement was assessed using pre- and post-tests, while anxiety levels were measured using the Hamilton Anxiety Rating Scale (HARS). The results showed a significant increase in participants’ knowledge (from a pre-test mean of 58.7 to a post-test mean of 85.3). Anxiety levels also decreased: 80% of participants experienced moderate to severe anxiety, which dropped to 7% after the intervention, with 60% reporting no anxiety symptoms. Ultimately, 100% of participants were able to perform the breathing exercise independently and stated they would continue practicing it regularly.
