Metode: Penelitian deskriptif dengan pendekatan studi kasus dengan penerapan manajemen varney meliputi pengumpulan data, Interpretasi Data, Diagnostik Potensi dan antisipasi, Tindakan segera, Rencana tindakan, Implementasi, Evaluasi dan Catatan Kemajuan dalam bentuk SOAP sebagai informasi dalam bentuk karya tulis ilmiah yang berjudul "Manajemen Asuhan Kebidanan Berkelanjutan pada Ny. R Klinik Rimenda Br. Tarigan, Kec. Medan Denai Kota Medan Tahun 2023" yang dilaksanakan pada tanggal 29 November-28 Juli 2022 menggunakan teknik pengumpulan data yaitu wawancara ibu, melakukan observasi, melakukan pemeriksaan fisik pada bayi, dan mengumpulkan data melalui rekam medis ibu dan bayi. Hasil: Dilakukan asuhan secara continuity dan didapatkan data bahwa selama hamil Ny.R rutin melakukan kunjungan antenatal 3 bulan terakhir menjelang persalinan, dan selama hamil tidak ada masalah atau komplikasi yang serius hanya keluhan seperti sesak nafas saat tidur telentang dan sering buang air kecill di malam hari, hal ini merupakan fisiologis pada ibu hamil trimester III. Masalah terselesaikan dengan baik dan tidak ada gap antara teori dan praktek. Kesimpulan: Berdasarkan apa yang diperoleh, penulis mengidentifikasi bahwa tidak ada kesenjangan antara teori dan praktek.
